Athletic Forms

Attention all 9th grade Student Athletes! If you are interested in earning Sports PE credit for participation in sports this year, please fill out the following Retroactive Sports PE Application (Class of 2027 only). In order to receive credit for 9th grade seasons, or any seasons moving forward you will need to pass 4 of the 5 fitness tests in PE this Spring and pass PE both semesters in 9th grade.  Students can earn up to 5 credits per school year.
